Underwriting condition
1. Please note the insured must be 1 - 60 years old, from the date of the insurance application
2. We do not cover individuals that:
- Have been diagnosed with COVID-19
- Have pre-exisiting conditions or illnesses due to COVID-19
- Are currently in quarantine due to COVID-19
- Have chronic obstructive pulmonary disease (COPD), chronic kidney disease (CKD stage 3, 4), and cardiovascular disease. cerebrovascular disease uncontrollable diabetes
3. For insured person who recovered from illness due to COVID-19 , doctor certificate confirmed recovery condition need to be presented when apply for this plan
4. This plan will only be effective if the insured follows the international travel advice by the WHO and the Department of Disease Control of Thailand
5. This plan is available for both Thai nationals and non-Thai citizens that reside in Thailand for at least 6 consecutive months prior to the insurance effective date and have a Work Permit
6. This plan covers side effects from COVID-19 vaccines that are registered with the Food and Drug Administration by the Ministry of Public Health in Thailand
7. This insurance plan covers benefits arising from post-vaccination side effects 90 days after each vaccination
8. Please note that there is a waiting period of 14 days before you can claim expense due to illness from COVID-19. This is a standard procedure for this type of insurance cover
9. The insured can only be covered under this plan and this must be their only COVID-19 / COVID-19 Vaccine Side Effects insurance with FWD General Insurance
